We use a chiropractic software that allows you to do your paperwork at home! When scheduling an appointment be sure to provide us with your email address. We will provide you with a direct link to our software to complete the necessary information.

In Office

If you would rather do the paperwork in the office, we provide tablets for you to use. If you need assistance with the tablet or paperwork we would be glad to assist you. Please arrive 10-15 minuets early to complete the intake information.
